Many Scout activities beyond the unit meeting require a parent permission and media release form. If you
are sponsoring an event, provide a copy of the Photograph/Recording Release Form with your
flyer and registration materials.

Use this form to propose a council-wide activity or event sponsored by your district but open to other
districts. Both community organizations and BSA unit leaders should submit one of these forms at least 6
weeks prior to a scheduled event for proper approval. Many activities should be planned 6 months to a year
in advance and this form begins the process. See the Activity Proposal tab to the left for more

Use this form to request development of an online reservation system for your event. The Council provides this
as a service to both Scout and community groups to assist in the efficient gathering and tracking of
interested participants.
